Introduction |
This screen lets the retail user to apply for opening a new operative account with the home bank. Account opening form is displayed to the user where he/she fills in the appropriate data that drives the opening of an operative account and submits the same to Relationship manager for further processing. This screen also lets the retail user to download the account opening form, so that he/she can download it, take a print out of the same and later on give it to a branch. |

Processing Date |
Select the date using the calendar widget to specify the date on which the account opening request has to be processed. |

Minimum Opening Amount |
Specify the minimum amount to open an account. Select the account currency from the drop-down list. |

Debit Account |
Select the debit account from the drop-down list. Note: Amount to the new account will be credited by debiting this account. Only operative accounts will be displayed in the drop-down list. |

Personal and Contact Details |

Name |
Specify the name of the account opener. |

Taxpayer ID Number |
Specify the tax payer identification number. |
|
Phone 1 & 2 |
Telephone numbers of the customer. Note: If in case the bank wants to contact customers regarding this request, bank can use this number. |

FAX 1 & 2 |
Fax numbers of the customer. Note: If in case the bank wants to contact customers regarding this request, bank can use this number. |

E-mail Address 1 & 2 |
Email addresses of the customer. |

Address |
Specify the customer’s address. |

City |
Specify the customer’s city. |

Postal Code |
Specify the customer’s postal code. |

State |
The name of the state. Select the state using Look Up button. |

Country |
Select the name of the country from the drop-down list. |

Specify remarks, if any. |

Debit Service Charge Account |
Select the debit account id for service charge. Note: If user does not select debit service charge account id from the drop-down list then, his/her primary account would be considered for the same purpose. |

Additional Information |
Displays the set of SLAs. For Example: It might take 7 working days for your request to be processed. |

Submit to Relationship Manager |
Submit the account opening request form to relationship manager for processing. Note: Clicking on this button will take the users to a screen where the above entered details are shown for confirmation and also user is prompted to enter the authorization details. Authorization details will be prompted only if the service request is authorization enabled. |
